Patient Portal

Our always-online patient portal service allows you to access important medical information whenever you want. 

• Review account balances
• Update personal information
• Request non-urgent appointments
• Review and request referrals
• Update medical and surgical history

You must be web-enabled at our office before you can access our patient portal. Once you have a valid email address on file, one of our staff members will issue you a unique user ID and password for the patient portal. 


Locked out of your patient portal?
In the event you forget or lose your username or password, please contact us to reset your login information. Call 330.334.6229 or email mdnorman@fpcwadsworth.com

Should you require a password reset, you will not be able to log in to your patient portal for a full 24 hours after the reset due to security purposes. Also, be advised that after three failed attempts with your login, the system automatically locks your account and you will need to contact the office.
